This article applies to all Diagnose users in the PI2 platform.
When selecting a survey to send to your team or organization, you may wish to ask questions that aren't covered in the available templates.
Use Diagnose's custom questions feature to fine-tune surveys to your liking and explore additional topics of interest to your organization.
How to add custom survey questions
Log in to the PI software.
Select Diagnose from the top navigation bar.
You will land on the Survey Center.
Select an existing survey draft (or select a new one).
Click the button titled "Add custom question."
You will be brought to the bottom of the survey screen.
Click "Add custom question."
A panel will open prompting you to add your custom question.
Select a question response type. You can choose between 2 different types:
Likert scale: Employees will be asked to agree or disagree with the given statement, on a scale of 1 (strongly disagree) to 5 (strongly agree).
Open response: Employees will provide a written response to the provided question.

Write your question. (For help crafting effective custom questions, read our best practices guide.)
Click "Add" to confirm.
A preview of your custom question will appear.
(Optional) Click the pencil icon to edit your custom question.
(Optional) Click the trash icon to delete your custom question.
Repeat as needed for any additional custom questions.
FAQs
Does Diagnose translate custom questions into other languages?
Does Diagnose translate custom questions into other languages?
No. In the near future, employees will be able to select their preferred language for Diagnose surveys. However, language translations only apply to PI's standard survey questions. Custom questions will remain in the language they were written in.
As a general best practice, we recommend writing custom questions in the primary language used by your organization.
