Creating custom survey questions

Learn how to create custom survey questions in PI Diagnose, and explore topics beyond what's covered in your survey template.

When selecting a survey to send to your team or organization, you may wish to ask questions that aren't covered in the available templates.

Use Diagnose's custom questions feature to fine-tune surveys to your liking and explore additional topics of interest to your organization.

How to add custom survey questions

  1. Log in to the PI software.

  2. Select Diagnose from the top navigation bar.

  3. You will land on the Survey Center.

  4. Select an existing survey draft (or select a new one).

  5. Click the button titled "Add custom question."

  6. You will be brought to the bottom of the survey screen.

  7. Click "Add custom question."

  8. A panel will open prompting you to add your custom question.

  9. Select a question response type. You can choose between 2 different types:

    • Likert scale: Employees will be asked to agree or disagree with the given statement, on a scale of 1 (strongly disagree) to 5 (strongly agree).

    • Open response: Employees will provide a written response to the provided question.

  10. Write your question. (For help crafting effective custom questions, read our best practices guide.)

  11. Click "Add" to confirm.

  12. A preview of your custom question will appear.

  13. (Optional) Click the pencil icon to edit your custom question.

  14. (Optional) Click the trash icon to delete your custom question.

  15. Repeat as needed for any additional custom questions.

FAQs

Is there a word or character limit for my questions?

No. There is no character or word limit for custom questions.

Can I add instructions or descriptions to my custom questions?

There aren't dedicated fields for question instructions or descriptions. That said, custom questions do not have a word limit. Feel free to include your instructions or description as part of the custom question itself.

How do I edit a custom question?

To edit a custom question:

  1. Click the pencil icon next to the question you'd like to edit.

  2. Make the desired edits to your question.

  3. Click "Save changes."

How do I delete a custom question?

To delete a custom question:

  1. Click the trash icon next to the question you'd like to edit.

  2. A success message will appear, confirming deletion.

Does Diagnose translate custom questions into other languages?

No. In the near future, employees will be able to select their preferred language for Diagnose surveys. However, these language translations only apply to PI's standard survey questions. Custom questions will remain in the language they were written in.

As a general best practice, we recommend writing each custom question so that it supports all languages in which your organization actively does business.

Example: You organization does business in 2 languages: English and French. If your survey contains custom questions, we recommend writing the question description in English and again in French, all as one question.

Here's how a Likert-scale question might follow this structure:

I have the resources I need to succeed in my job / J'ai les ressources nécessaires pour réussir dans mon travail

  • Strongly agree / Tout à fait d'accord

  • Agree / D'accord

  • Neither agree nor disagree / Ni en désaccord ni d'accord

  • Disagree / Pas d'accord

  • Strongly disagree / Pas du tout d'accord

Can I preview what employees will see when taking the survey?

Yes. To preview your employees' view of the survey:

  1. Scroll back up to the top of the survey page.

  2. Click the button titled "Demo Survey."

  3. A preview of your survey will open in a new window.

  4. On the final page of the survey, you will see any custom questions listed under the header, "Additional questions from your organization."
