Creating custom 360 feedback questions

Want to fine-tune 360 feedback questions or make new ones altogether? Perform makes the process easy.

How to make custom 360 questions

Creating custom questions starts with creating a custom 360 template.

To do so:

  1. Log in to Perform.
  2. Click “Feedback” on the left-hand side.
  1. Select the 360 Feedback tab at the top of the screen.
  1. Click “Templates.”
  1. Click the “Create template” button.
  1. Provide a name and description for your template.

Next, you’ll want to create the actual questions. Your custom template includes three different question groupings:

  • Self questions: These are questions the 360 recipient will answer about their own experience.
  • Peer questions: These are questions feedback contributors will answer about the recipient.
  • Summarizer questions: These are questions designed to help the summarizer (a manager, etc.) finalize the 360 at the end of the process.

To create a new question:

  1. Select a question grouping.
  1. Click the “Add content” button.
  1. Select your content type from the dropdown options.
  1. Enter the question and any related items.
  2. (Optional) Set the question as required.
  1. (Optional) Click “Add content” to repeat the process.
  2. When finished with a grouping, click “Back to template.”
  1. Repeat the process with the other two groupings.
  2. Finalize your questions by clicking “Save & Exit” in the top-right corner.

Note: To use the template, you must add at least one question to each of the three question groupings.

How to use your new 360 template

  1. Click “Feedback” on the left navigation bar.
  2. Select the 360 Feedback tab.
  3. Click either “Kick off Self 360” or “Kick off 360 Feedback.” (The latter option only shows if you’re a manager in Perform.)
  1. Select your new template from the dropdown options.
  1. Click “Start” to proceed.
  2. Continue the 360 feedback process as normal.

FAQs

I’m not sure what questions to create. Is there a recommended starting point?
Yes! We recently released a “Standard 360” template that you’re free to use as is or modify to fit your needs.

To modify our template:

1. Click “Feedback” on the left navigation bar.

2. Select the 360 Feedback tab.

3. Click “Templates.”

4. Look for the template titled “Standard 360.” (It will have a label titled “Default.”)



5. Click the ellipsis icon.

6. Click “Duplicate Template.”

7. Modify the name and description.

8. Click into any of the question groupings.

9. You’ll see prepopulated questions you can modify to your liking.
I don’t see my custom template in the dropdown options. Why?
To use your custom 360 template, you must add at least one question to each of the three question groupings:

– Self questions
– Peer questions
– Summarizer questions
